  • Website - Ciccone/McKay Financial Group may collect certain information about you when you register to become a member of the Website. In addition, Ciccone/McKay Financial Group keeps a record of the email addresses of those who communicate with us via our online Feedback tool. Ciccone/McKay Financial Group also collects both aggregate and user-specific information on what web pages consumers access or visit, and information volunteered by consumers, such as survey information and contest application information, as is further described below.

 

  • Client Onboarding – Ciccone/McKay Financial Group may collect certain personal information from you when you become a client or make use of our Services. Depending on the type of Service we are providing you with, this information may include your first and last name, home and mailing address, email address, date of birth, social insurance number, identification, e-signature, gender, and beneficiary information (if applicable).

 

  • Purchases - Ciccone/McKay Financial Group may also collect information about you when you purchase products from us by telephone, mail, fax or on-line, such as those relating to insurance, investment, investment referrals, and/or deposits of any kind.   • Financial Planning – If you take advantage of Ciccone/McKay Financial Group’s financial planning services, we may collect certain personal information related to your financial background in order to understand your financial needs and tailor our Services.   • Complaints/Requests - Ciccone/McKay Financial Group may collect information about you if you contact Ciccone/McKay Financial Group with a consumer complaint or request.   • If you set up an individual pension plan or a retirement compensation agreement with one of our corporate clients, we will collect your first and last name, email, phone and home address, social insurance number, date of birth, T4 income details (past and present), RRSP information, IPP investment information.

 

  • If you decide to obtain insurance services or products, we will collect your first and last name, email, phone and home address, income information, net worth, and health, medical and lifestyle details in order to assist you in obtaining such insurance services or products through a third-party insurer.

 

  • If you require investment management services, we may collect or access certain information from your investment management service provider including your first and last name, email, phone, and home address, date of birth, social insurance number, portfolio statements, income information and net worth in order to better assist you with your financial planning needs.

 

  • In connection with The Immix Group Employee Benefits Ltd.’s employee benefits services, we may collect your first and last name, date of birth, home address, dependent information, occupation, salary (in some cases), email address, and phone number in order to administer such services on behalf of our clients.
